I suggest you download and print this chart made by myself as well as the list of references and the quantities of 11/0 miyuki beads (preferably) used to make this weaving.

Dimensions of the model finished in 11/0 miyuki beads: 6 cm high = approx 2.5 inches

To make this pattern, you will need to have miyuki beads, thread and a needle in stock. You will also need to know the technique of brick stitch or peyote.
I personally used the brick stitch to make this model; you will find many tutorials on You Tube, this one being particularly explicit to start:
